Fax or mail both forms and a copy of your Photo ID to the fax number or address on the Credit Application and use the vendor data at the bottom of the Order Form. Feel free to call or e-mail if you need help filling these out.

Personal/Consumer Lease
For private individuals in situations where the leased equipment has no business-related connection .

Personal/Business Lease
For businesses that are just starting up, businesses in existence for less than six months, and private individuals that have a business purpose or tie-in with the leased equipment.

Business Lease
For businesses that have been in existence for greater than six months, have established a business bank account and have trade references.
